1. Download the “Sam by UCM Digital Health” app.
Update your app if it is already installed to get the latest version.
2. Click “Create an account.”
3. Enter your email address and phone number, then click the “Send Verification Email” button.
Note: the information you enter must match what your the employer has on file, including the spelling.
4. Check your inbox for a message from UCM Digital Health with a one-time passcode and instructions for finishing your account set-up.
5. Head back to the Sam app, enter the code provided, and start accessing care today!
